Med Spa Ads in 2026: 63 Appointments in 3 Weeks at €3.37 Per Lead

A med spa ditched Instagram boosting for real paid ads. Result: 63 appointments, 26 closed clients, 30.66× ROAS in 3 weeks. Full breakdown inside.

Most med spa owners we talk to are running "marketing" that doesn't actually run. Word of mouth. The occasional boosted Instagram post. Maybe a referral program that works when one good client tells two friends.

Then a slow month hits. The calendar that was full in March empties out by mid-April. You boost a post. You drop a price. You hope the algorithm gods show your story to someone with €600 to spend on filler.

Likes come in. Zero patients book. The trap is calling that "advertising." Boosting and running real ads are not the same thing. They use the same platforms, but they're built on completely different systems. One sends your post to strangers and hopes for the best. The other sends a specific offer to a specific audience with a specific path from "saw the ad" to "in your chair." We just ran the second kind for a med spa in Nice, and three weeks later her dashboard had 63 appointments booked, 26 closed clients, and a 30.66× return on ad spend.

The Story (Real Numbers, Real Client)

Before we started with Holistic Bien Être, her marketing was the same as yours probably is right now. Posts on Instagram. Boosted occasionally for €20-30. Word of mouth. A few likes per post and a handful of new patients per month who heard about her from a friend.

Day 1 of the new system: €2 in spend. First appointment booked 1 hour and 27 minutes after launch. That moment told us the offer, targeting and funnel were dialled in. The rest was about feeding the machine.

Three weeks in, here's what was on her screen:

In this industry, "appointment" doesn't mean "we'll talk." It means a patient who's decided to spend money. The closed-client number is what most agencies hide behind impressions and CTR. We don't.

Why Boosted Posts Can't Do This (5 Reasons)

The difference between boosting a post and running ads is the system around the spend. Same algorithm. Same auction. Same CPM. Different outcomes — by 30×.

1. Boosting has no real targeting. When you click "Boost," Instagram shows your post to people who look vaguely like your followers, plus some Facebook spillover. It's not built for "women aged 28-55 in a 15km radius around your clinic who follow aesthetic medicine accounts and have engaged with similar offers in the last 90 days." That's an actual ads-manager audience. Boosting can't build it.

2. Boosting amplifies whatever you already posted. If your existing post is "loving this new treatment," boosting it pays Instagram to show that to strangers. Strangers don't book appointments off vibes. Real ads start with an offer — "Free aesthetic consultation this week, 12 spots open" — and a single ask. The post on your feed is for people who already know you. The ad is for the person who's never heard of you and needs a reason to click in 1.5 seconds.

3. Boosting has no funnel. A real ad sends the click to a landing page or a booking form built for one purpose: turn the click into a contact. Phone, email, calendar slot. Boosted posts send the click to your Instagram profile, where the visitor can scroll, get distracted, and leave. We've seen the same offer convert 4× higher when the click goes to a 1-question booking page instead of a profile feed.

4. Boosting has no follow-up. When 126 leads come in over 3 weeks, you can't call all of them in real time. The 41% of inbound calls that med spas miss every month is the biggest leak in the industry — and boosting doesn't help you fix it. Real ad systems include a missed-call text-back and a sequenced follow-up that recovers the leads you'd otherwise lose. Without that, you'd lose half of those 126 leads to silence.

5. Boosting tracks the wrong things. Reach. Likes. Engagement. None of those pay rent. Real ad campaigns track cost per lead, cost per appointment, cost per closed client, and ROAS. When you can see which ad set is closing patients at €40 per close vs €120 per close, you double down on the winner. Boosting gives you a vanity dashboard with no signal.

What "Real Ads" Actually Means

Calling something "advertising" doesn't make it so. Real ad campaigns for med spas have four moving parts, all working together:

  • Offer: a specific promise — free consultation, discounted intro session, package deal — with a deadline or capped capacity. Without an offer, you have a brochure.
  • Targeting: demographics, geography, interest stack, and lookalike audiences built from your best existing patients. Boosting can't do this. Ads Manager can.
  • Funnel: a one-purpose landing page or booking form. Strip everything that isn't the appointment. The fewer choices, the higher the conversion.
  • Follow-up: automated SMS within 30 seconds of a missed call, a real human callback within 2 hours, and a 5-day sequence to keep warm leads warm. Speed wins.

This is why agencies that "boost your social media" can't get you Holistic-style numbers. They're playing a different game with the same logo on the dashboard.

Before you spend another euro on Instagram, answer these three questions honestly:

  1. Do you know the cost per booked appointment your current ads (or boosted posts) are producing?
  2. When someone calls your clinic and you don't pick up, what happens — voicemail, or an automated text-back within 30 seconds?
  3. If your spend doubled tomorrow, do you know exactly which campaign would scale and which one would break?
